- 요약Substance use remains a public concern due to its popularity and health consequences. Further, polysubstance use leads to severe consequences. The current dissertation study aimed to investigate two crucial issues regarding the mechanism of substance use and the trajectories of polysubstance use. This dissertation consists of two studies. Study 1 investigated how adverse childhood experiences (ACEs) and substance use behaviors in early middle adulthood are connected through a sequential psychological pathway from internalizing problems in young adulthood to externalizing problems in adulthood. Study 1 used data from the National Longitudinal Study of Adolescent to Adult (Add Health) Waves I -V (1994-2018). Adolescents aged 18 or younger at Wave 1 were selected (N=2,308). Generalized structural equation modeling (GSEM) was conducted to investigate the sequential psychological pathways. Study 2 investigated the longitudinal trajectories of polysubstance use and their associations with internalizing problems and externalizing problems. Adults aged 18 and older (N=16,203) were extracted from the Waves 1-5 Population Assessment of Tobacco and Health (PATH) Study. Group-based trajectory modeling (GBTM) was first performed to identify the trajectories of polysubstance use. Weighted multinomial logistic regressions were then conducted to investigate the associations between internalizing problems, externalizing problems, and the trajectories of polysubstance use.Study 1 revealed how ACEs and substance use behaviors in early middle adulthood are connected through a sequential psychological pathway from internalizing problems in early adulthood to externalizing problems in adulthood. ACEs were significantly associated with substance use behaviors in early middle adulthood, including cigarette use, marijuana use, and cigarette and marijuana co-use. To prevent individuals who are exposed to ACE from engaging in substance use, intervention efforts should tailor strategies and implement preventive education based on different stages of life course to interrupt this long-term continuity. Integrating therapeutic strategies into programs may also be beneficial.Study 2 was the first study that identified the U.S. national polysubstance use trajectories from 2013 to 2019 and their associations with internalizing and externalizing problems. The results of group-based trajectory modeling reveal five polysubstance use trajectory groups. Given that approximately 4 in 10 adults were in high-risk polysubstance use groups, polysubstance use warrants continued public health attention. Understanding the longitudinal polysubstance trajectories is crucial in mitigating substance use epidemic, including informing prevention efforts and providing the basis for designing long-term strategies. Internalizing problems and externalizing problems predicted high-risk polysubstance use trajectories. Intervention efforts are warranted to target at-risk populations and implement interventions to intervene in internalizing and externalizing problems in order to prevent polysubstance use and associated health consequences. Further, health providers that implement substance use prevention should be aware of how polysubstance use can be intertwined with internalizing and externalizing problems.
