초록/해제
요약Postmenopausal women with breast cancer on aromatase inhibitor (AI) therapy are vulnerable to diminished health-related quality of life (HRQoL). The first two study aims examined the relationship between AI treatment and HRQoL over an 18-month course of therapy. The third aim explored work-related factors, including significant employment events and job characteristics, as predictors of HRQoL. Data from the "Long-Term Trajectory of Cognitive Function Related to Anastrozole Use in Women (R01CA107408)" study were used for this secondary analysis. The participant sample (n=351) consisted of two AI cohorts (chemotherapy followed by AI therapy and AI therapy only) and a control group. Study variable selection was guided by the domains of disease/treatment effects, individual/interpersonal factors, and work environment presented in the Cancer Survivorship and Work Model. A total of eight linear mixed-effects models were constructed to examine the study aims for four SF-36 HRQoL components: Role Physical and Role Emotional scales and the Physical and Mental Component Summary scores. Goodness of fit tests were significant across all of the final models however the set of significant predictors varied. Skill discretion and decision authority job characteristics were significantly associated with HRQoL in the Role Physical and Mental Component Summary models. The interaction between treatment group and time was significant in four of the eight component models. These findings suggest the need for comprehensive multidisciplinary cancer survivorship programs to address the multi-pronged nature of factors that affect different aspects of HRQoL differently in this population.
