- 요약Infectious diseases cause a significant burden of illness and mortality in healthcare settings among patients and healthcare workers (HCW). This dissertation aims to add to the knowledge base about how to reduce risk for HCW exposure to pathogens and prevent occupationally acquired infectious diseases. Personal protective equipment (PPE) is a primary strategy to prevent HCWs' exposure to pathogens, but HCWs can be exposed to pathogens on PPE during the PPE doffing process. A scoping literature review was performed to assess patterns and determinants of body contamination after PPE doffing by HCWs. Nineteen studies were reviewed. All studies reported some post-doffing contamination on HCWs, which was most frequently observed on the hands, wrist, face, and neck. While some studies identified determinants of post-doffing contamination on HCWs, it was not possible to compare these findings across studies owing to the variety of study methods, doffing protocols and PPE studied.Norovirus is linked to hundreds of outbreaks of acute gastroenteritis among patients and HCWs in healthcare facilities every year. There is a need to establish evidence-based cleaning protocols to remove inactivate norovirus on environmental surfaces after a vomiting event. An experimental simulation study was performed to quantify the magnitude and extent of environmental contamination due to vomiting in order to inform the spatial area for cleaning. In this experiment, a vomiting system was used to simulate fluorescein-containing vomitus. Four experimental design factors believed to influence vomiting dispersion - head angle, vomitus volume, ejection pressure, and vomitus viscosity - were tested using a blocked randomized design. The interpolated average extent of two-dimensional dispersion was 45 m2, spanning a width of 6.6 m and a length of 6.6 m from the source. These results affirm current recommendations to clean at least 25 ft (7.6m) in length from a vomiting event.The quantitative microbial risk assessment (QMRA) framework was applied to describe how to determine the infection risk from cleaning up of norovirus-containing vomitus by environmental service workers (ESWs). This description demonstrates that while some data gaps and uncertainties exist, it is possible to perform a QMRA for this scenario. Future work will implement this analysis.
