요약Pediatric obesity is a public health crisis associated with costly cardiovascular and chronic diseases and decreased quality of life. Family-based treatments for childhood obesity have demonstrated effectiveness but struggle with high participant drop-out and inconsistent attendance, limiting opportunities for intervention effects and improving health outcomes. Due to the complexity of pediatric obesity, contributors to children's weight gain can greatly vary from family to family. Individually tailored family-based programs use rigorous assessment to develop individualized treatment plans that address a family's unique needs and treatment priorities. The Family Check-Up 4 Health (FCU4Health) is an assessment-driven individually tailored family-based pediatric obesity program that targets parenting skills and health behaviors and has had high rates of participation and retention among a low-income predominantly ethnic minority sample. Despite the promise of individual tailoring, tools to quantify the process of individual tailoring are understudied, hindering the ability to evaluate individual tailoring and test the theory that tailoring affects program engagement and outcomes. In this dissertation, I develop the Observational Assessment Tool for Tailoring (OATT) to measure interventionist fidelity to tailoring in pediatric obesity interventions. The first study conducted a scoping review of tailored pediatric obesity interventions to understand to current state of the literature. Results exhibited a range of uses and varied definitions of tailoring in pediatric obesity interventions including some that deviate from individualized designs. In the second study, I developed the empirically-driven OATT based on the research concerning tailoring, pediatric obesity, and behavior change. Next, video recorded sessions and data from a trial of the FCU4Health, the Raising Healthy Children (RHC) project, was used to pilot the OATT and exhibit preliminary validity and reliability. Using a second trial of the FCU4Health, the Healthy Communities 4 Healthy Students (HC4HS) project, I trained three new coders and tested the RHC model. Results showed a two-factor model with good fit statistics and good inter-rater reliability for RHC; however, the HC4HS data were not suitable for factor analysis based on tests of sampling adequacy. The HC4HS sample was partially recruited during the COVID19 pandemic, and related factors possibly affected the session content. In the third study, scores from the new OATT rating system were used in path analysis to test a theory of implementation research examining the relationship between fidelity to tailoring, program engagement and changes in health behaviors using a combined RHC and HC4HS sample. Results showed that OATT dimensions were associated with FCU4Health program engagement. Although the data are preliminary and require further testing, the OATT shows potential for improving research regarding tailored pediatric obesity interventions and program fidelity.
