- 요약Gastric cancer is the fifth leading cancer, the leading infection-associated cancer, and the fifth leading cause of cancer mortality globally. There is significant heterogeneity in rates of gastric cancer incidence across populations and geographies. Previous research has identified biological, hereditary, behavioral, environmental, and pathogenic risk factors for gastric cancer, all of which interact and modulate risk of disease. The bacterial pathogen Helicobacter pylori has been established as the primary risk factor for gastric cancer, with an attributable risk of 75-88%, as one to five percent of H. pylori infections progress to gastric cancer. However, given that H. pylori infection as a primary cause is neither a necessary nor sufficient cause of gastric cancer, there remains a significant knowledge gap of the ecology of the disease and pathogen. This dissertation investigates the disease ecology of gastric cancer and H. pyori in western Honduras, which has endemic H. pylori infection and one of the highest gastric cancer rates in the western hemisphere and world. I conceptualize the ecology of gastric cancer as the integration of two disease ecologies: primarily driven by H. pylori infection, which has a disease ecology of its own; and also by other factors such as genetic mutations, environmental exposures, and behavioral factors. This dissertation project builds on previous work in medical geography to expand the integration of landscape genetics in disease ecology by investigating how pathogen genetics and phenotypic expression may vary across space. Using data from a population-based cancer registry and population-based controls, this project presents insights from three empirical papers that advance the understanding of integrated pathogen and cancer ecology. First, I analyze how proteins produced by H. pylori associate with histological sub-types of gastric cancer to describe how gastric cancer risk varies based on the characteristics of the bacteria. Second, I investigate the environmental drivers of H. pylori infection to characterize how infection risk varies based on environmental and behavioral factors. Finally, I model the associations between population-level characteristics with gastric cancer within the study area to show the geographic arrangement of excess cancer risk for sex and histological sub-type strata. The findings of this dissertation demonstrate how a health and medical geography approach to the study of a complex disease refines risk evaluation across a representative region, and these insights can be used to inform practices for risk screening and preventative care in a high-risk population.
