요약Amyotrophic Lateral Sclerosis (ALS) is a neurodegenerative disease that causes muscle weakness, loss of voluntary motor control, and respiratory problems. Present clinical metrics of ALS remain subjective, suffer from high variability, or require invasive biopsies. Magnetic resonance imaging (MRI) offers the ability to provide quantitative imaging biomarkers and a non-invasive way to monitor and assess ALS progression and structural changes. Diffusion MRI in particular has potential for this purpose due to its sensitivity to microstructural properties. This thesis seeks to evaluate the usefulness of diffusion MRI in skeletal muscle for imaging of ALS patients.This works starts by applying a diffusion tensor imaging (DTI) sequence in the leg of a rat model of ALS. Four ages of rats (corresponding to stages of disease progression) were imaged to assess the ability of diffusion MRI to monitor structural changes secondary to ALS. Results were compared to histological analysis to provide insight into which morphometric features drove the sensitivity of diffusion metrics to ALS. The subsequent studies revolve around testing in healthy controls the repeatability of MR sequences which will eventually be implemented in ALS patients as part of an R61 study. First, the repeatability of a diffusion kurtosis tensor acquisition and the resulting diffusion parameters in the lower legs was validated. Then, since bulbar symptoms are common in ALS, the repeatability of a full bulbar imaging protocol including T1 mapping, T2 mapping, T2* mapping, dynamic T2* mapping, and diffusion kurtosis imaging was analyzed.
