- 요약Background: Moderate to vigorous physical activity (MVPA) is associated with improved breast cancer survival, yet few studies have assessed the demographic, social, and clinical factors of MVPA across a diverse group of survivors. Black women report lower MVPA levels, are diagnosed with more aggressive cancers, and have overall higher mortality. Breast cancer outcomes could be attributed to biological markers (e.g., immune response and risk of recurrence score), although the role of MVPA in these pathways is unclear. Understanding patterns and correlates of MVPA in a diverse population of breast cancer survivors may improve recommendations and treatment allocation.Methods: This project leveraged the Carolina Breast Cancer Study Phase 3 (CBCS3; Aim 1: Longitudinal study Naim1=2,994; Aim 2: Cross-sectional study Naim2=1,424). CBCS3 is a population-based study that oversampled black and young women (50 years). This project had two aims: 1) to describe MVPA patterns and their correlates, and 2) to examine associations between pre-diagnosis MVPA and tumor molecular signature overall and stratified by estrogen receptor (ER) status.Results: In Aim 1, 83% of participants who reported MVPA pre-diagnosis experienced a decline at 6-month post-diagnosis. However, a higher proportion (85%) of these participants rebounded their MVPA at 18-month post-diagnosis compared to those who reported no MVPA pre-diagnosis. Demographic, social, and clinical correlates such as income $30K, community-level SES, and chemotherapy were significantly associated with A) higher likelihood of reporting any MVPA at 6- and 18-month post-diagnosis and B) either no change or increased MVPA. In Aim 2, pre-diagnosis MVPA was not associated with adaptive immune response or risk of recurrence score incorporating proliferation and tumor size (ROR-PT) in the overall cohort. However, among ER-negative cases, pre-diagnosis MVPA was associated with a lower frequency of adaptive immune response. Although not statistically significant, MVPA was associated with a lower frequency of high ROR-PT in ER-positive cases and a high frequency of high ROR-PT in ER-negative cases.Conclusions: Demographic, social, and clinical correlates of MVPA were identified among breast cancer survivors. Pre-diagnosis MVPA was indicative of MVPA participation at post-diagnosis and was associated with a lower frequency of adaptive immune response among ER-negative cases.
