- 요약Growing evidence shows that opioids have been over-prescribed for patients returning home after surgery in the U.S. Currently there are no consensus guidelines on optimal post-surgical opioid dosing at discharge for pain control following various surgeries. Some observational studies suggest that opioid use is associated with reduced kidney function, highlighting the need to investigate the association between opioid prescription post-surgery and kidney dysfunction after discharge. The overarching goal of this dissertation is to evaluate opioid dosing strategies in a post-surgical setting and subsequent opioid use disorder (OUD) and changes in kidney function. The objective of this dissertation research is using real-world data to 1) Compare the impact of 2016 CDC opioid guideline and 2018 Minnesota opioid guideline on reducing high total morphine milligram equivalents (MME) prescribing rate at post-surgical discharge; 2) Evaluate the association between high/low total MME and the risk for developing opioid use disorder for opioid naive adult patients; and 3) Evaluate the effects of high versus low total MME prescribed at post-surgical discharge on change in kidney function over time after surgery in one year.
