요약In this thesis, we investigate insomnia symptoms among men newly diagnosed with advanced prostate cancer. We leveraged a unique dataset with measures repeated every three months over a two-year period. First, we examined patient-reported insomnia symptoms (difficulty falling asleep, difficulty maintaining sleep, and nonrestorative sleep) and the association between common prostate cancer treatments and their insomnia complaints. We found that difficulty falling asleep, difficulty maintaining sleep, and nonrestorative sleep were highly prevalent among this population, and that prostate cancer treatments such as androgen deprivation therapy and androgen receptor signaling inhibitors were associated with increased insomnia, independent of other common symptoms such as pain and nausea. We conclude that insomnia should be treated independently of other symptoms experienced during cancer treatment.Next, we examined the association between patients' insomnia symptoms and patient-reported quality of life and other patient-reported symptoms over the two-year follow-up period. Overall, we found that insomnia complaints among men with advanced prostate cancer were significantly associated with emotional and cognitive functioning and fatigue. However, among black men, worse insomnia was associated with worse physical domains such as role functioning, pain, and fatigue, while among white men, insomnia was associated with worse psychological domains, such as emotional and cognitive functioning. These findings underscore the complex interplay between insomnia and quality of life in diverse populations of men with advanced prostate cancer.Finally, we characterized the longitudinal trajectory of insomnia symptoms among men with advanced prostate cancer, investigated patient and tumor characteristics associated with insomnia symptom trajectories, and evaluated the association between insomnia symptom trajectories and patient mortality. We found that most men had persistent insomnia symptoms over the course of the study. Additionally, we report that poor trajectories of difficulty falling asleep were associated with an increased risk of overall and prostate-specific mortality, but difficulty maintaining sleep and nonrestorative sleep were not associated with survival. This thesis emphasizes the unaddressed problem of insomnia among men with advanced prostate cancer and highlights the potential importance of treating insomnia among these men, as well as the need for further investigation of the impact of insomnia in this patient population.
