- 요약Mexico was the first country in the Latin American region to ratify the Framework Convention for Tobacco Control in 2004.1,2 From 2004 to 2011, the Mexican government implemented tobacco control policies including tobacco tax increases, local smoke-free laws, and pictorial health warning labels,3 but then, there were no additional national tobacco control policies until 2019,4 when tobacco tax increases were adjusted automatically for inflation.5 In 2022, Mexico implemented a law that bans the advertising of tobacco products, including at points of sale, and a comprehensive smoke-free law at the national level.4,6 However, the tobacco industry continues to adapt to the new regulatory environment, including the introduction of flavored capsule cigarettes into the Mexican market in the early 2010s. Despite a decrease in current smoking between 2002 and 2009, the prevalence of current smoking has changed little since flavored capsule cigarettes were introduced to the market in Mexico, with the prevalence of current smoking of 16.5% in 2009 and 15.6% in 2023.7 However, little is known about the disparities of flavored capsule cigarette use at the intersection of multiple sociodemographic variables, and no studies in Mexico have evaluated the potential effectiveness of banning flavored cigarettes and how this prohibition could influence public health outcomes. The goals of this dissertation were to: (1) To examine disparities in flavored capsule and non-flavored capsule cigarette use at the intersection of sex, age, wealth, and geographic region among Mexicans aged 15 years, (2) To predict the potential impact of the flavored cigarette ban on the choice of cigarettes, e-cigarettes, and heated tobacco products, and (3) To simulate the potential public health impact of the potential flavor cigarette ban on the prevalence of smoking and in the number of premature deaths attributable to smoking.In Aim 1, I described flavored and unflavored capsule cigarette use patterns at the intersection of age, sex, region, and wealth. This study reveals that current cigarette use is still high among some specific subpopulations in Mexico, and most of these subpopulations are using flavored capsule cigarettes in high proportions, including adolescents, young male adults with low and medium wealth, and young female adults with high wealth. In Aim 2, I designed and conducted an online survey and a discrete choice experiment to assess potential individual changes in smoking and e-cigarette use patterns under a hypothetical flavored cigarette ban. Results from both the online survey and the discrete choice experiment show that cigarette use would decrease under the potential flavored cigarette ban due to reductions in smoking initiation and increases in smoking cessation. Moreover, the majority of adults who smoked cigarettes were not opposed to a flavored cigarette ban. Finally, in Aim 3, I adapted and developed a simulation model to estimate the potential public health benefits of a flavored cigarette ban, examining the impact on smoking prevalence and the number of premature deaths attributable to tobacco that can be potentially saved under a ban. I used inputs from the Aim 2 online survey and the discrete choice experiment to modify initiation and cessation probabilities and inputs from Mexican national surveys. From the simulation model, I estimated that implementing a flavored cigarette ban has the potential to decrease smoking prevalence by 42.9% in women and 27.0% in male adults by 2060, and approximately 170,000 smoking-attributable deaths could potentially be prevented.
