- 요약Antimicrobial resistance (AMR) is a major threat to public health, as AMR infections are more difficult to treat, are more likely to lead to complications, and have been increasing in prevalence. AMR infections are estimated to have contributed to 4.95 million deaths in 2009 alone.1 Emergence of extended spectrum beta-lactamase (ESBL) producing bacteria, primarily E. coli, are particularly concerning because they are resistant to several classes of antibiotics, including some last-line antibiotics, third-generation cephalosporins. Urinary tract infections (UTI) are an important focus for AMR research, as one of the most common bacterial infections. The majority of UTIs are caused by uropathogenic E. coli (UPEC) in both community and healthcare settings. E. coli are incredibly diverse and readily share plasmids, aiding in the dissemination of AMR genes. Surveillance of UPEC to prevent and diagnose AMR infections is thus an important component of the fight against AMR.The work presented here leverages genomic and epidemiologic methods to address two issues related to AMR infection surveillance. The first chapter provides an introduction to the complexity of AMR and UPEC. The second chapter uses publicly available whole-genome sequence data to develop a rapid, inexpensive assay for identifying common UPEC sequence types, to aid in future epidemiologic surveillance for the control of UPEC. The third, uses electronic health record data to evaluate the risk of developing complications from a community onset UTI caused by ESBL-producing E. coli, emphasizing the importance of early detection and monitoring of ESBL infections.
