- 요약Patients who sustain anterior cruciate ligament injury and undergo reconstruction surgery (ACLR) experience persistent impairments including muscle weakness and proprioceptive dysfunction, leading to functional limitations and risk of secondary injury. According to the Dynamical Systems Theory, ACLR and the ensuing impairments are perturbations to the motor system that may manifest as changes in movement coordination between joints. The purpose of this research study was to examine differences in lower-extremity inter-joint coordination during performance of a cutting maneuver between patients who had ACLR and uninjured participants. It was hypothesized that hip-knee joint coordination would be less anti-phase and knee-dominant and knee-ankle joint coordination would be more in-phase and ankle-dominant in patients who had ACLR compared to uninjured participants.Twenty-two patients who had ACLR and 15 uninjured participants performed 5 repetitions of a 45-degree cutting maneuver while hip, knee, and ankle sagittal-plane joint angles were collected. The joint-angle data from the involved leg of patients and dominant leg of uninjured participants during the braking phase, defined as the time from the pivot foot landing on the floor to peak knee flexion, were used to calculate coupling angles to represent inter-joint coordination. Coupling angle was interpreted as two joints moving in-phase (in the same direction) versus anti-phase (in opposite directions) and proximal- versus distal-dominant (proximal versus distal joint moving more than the other). Circular statistics were used to compare differences in mean hip-knee and knee-ankle coupling angles between patients and uninjured participants.Hip-knee coupling angle was not different between groups and both groups exhibited an anti-phase (hip extension and knee flexion) pattern throughout the braking phase. Knee-ankle coordination was in-phase and more ankle-dominant in the patients compared to the uninjured participants. This may represent emergence of a new coordination pattern in patients who had ACLR. Supplementary analysis revealed three distinct landing strategies, which explained the high variability in coupling angle in the beginning of the braking phase. Both groups exhibited similar levels of variability in inter-joint coordination, suggesting inherent variability when individuals perform a cutting maneuver. The findings of this research study may have implications in sports performance and future injury risk.
