초록/해제
요약The healthcare environment has continued to be reshaped and forced to evolve over the last several decades and even more so in recent years. This evolution and reshaping have been forced by both internal and external factors withing the healthcare landscape. This has tasked hospital leaders with exploring new ways of delivering care. The COVID-19 pandemic was an external circumstance the forced hospital leaders to look at alternate ways of providing care, more specifically in the home setting. Hospitals were introduced to the concept of acute care in the home by CMS and for many they adopted this model to meet the needs of their hospitals, communities, and patients. There remains to be a lack of research and knowledge on the topic of hospital in the home. The purpose of this study was to evaluate the organizational characteristics and market factors associated with adoption of the model and ultimately provide a better lense of what these hospitals look like. Rogers Diffusion of Innovation served as the leadership theory and the CFIR Framework provided a model for further understanding of the facilitators and barriers for adoption. A multivariate binary logistic regression was the model chosen to analyze and provide results on the organizational characteristics and market factors that are associated with greater adoption of hospital at home. For organizational characteristics, hospital structural and operational characteristics provided the most support, whereas financial characteristics did not show support. As it relates to market factors, both competitive and physical factor variables were split on support, and socio-demographic showing support for both variables. The finding and results suggest that there are predictors to the model and that organizational factors might provide the largest grouping of variables for hospital leaders to be informed of. In addition, the interviews with hospital leaders provided further knowledge related to facilitators and barriers for adoption of the program.
