요약Despite lifespan increases among individuals with Down syndrome (DS), nearly all will develop Alzheimer's disease (AD). Yet, the timing and mechanisms through which AD risk and protective factors emerge, and the extent to which modifiable lifestyle factors may protect against decline, remain poorly understood. This dissertation investigates the biological and lifestyle correlates of AD progression in adults with DS, with a focus on body mass index (BMI), systemic inflammation, and physical activity.Paper 1 examined longitudinal changes in BMI in relation to AD pathology and cognition. Findings indicated that adults with DS begin to experience BMI decline in their early 40s, with greater decline predicted by early amyloid beta (Aβ) burden and associated with memory decline and increased dementia symptoms. Paper 2 explored associations between obesity, age, Aβ, and systemic inflammation. Results revealed distinct, but overlapping inflammatory markers linked to obesity and aging, whereas Aβ was not robustly associated with systemic inflammation, highlighting the need for neuroinflammatory markers (i.e., YKL40, GFAP) in this population. Paper 3 evaluated whether moderate-to-vigorous physical activity (MVPA) conferred resistance to Aβ burden or resilience to cognitive decline. MVPA did not reduce amyloid accumulation but moderated its impact on cognition (i.e., there was a resilience pathway), such that adults with higher activity levels experienced less cognitive decline despite similar pathology.Together, these studies suggest that BMI decline may be an early indicator of AD in DS, that systemic inflammation is associated with obesity and age, and that physical activity supports cognitive resilience in the face of AD pathology. Findings underscore the importance of monitoring physical health indicators and promoting active lifestyles to improve outcomes for adults with DS as they age.
