요약This dissertation explores a structured educational intervention aimed at enhancing confidence and knowledge retention among phase II cardiac rehabilitation patients through the "teach back" method. Drawing from a semi-structured interview with a former patient, fishbone diagram and empathy interview, insights regarding the need for structured education in cardiac rehabilitation and the missed opportunity for improved patient engagement were gained. This study proposed a 12-session individualized education program designed to equip phase II cardiac rehabilitation patients with the knowledge, skills and confidence to excel in their cardiac health maintenance in phase III cardiac rehabilitation. The program aims to foster a more holistic care approach, improving patient outcomes and reducing clinician burnout. The proposed changes were assessed through 3 key inquiry questions: 1. Are phase II cardiac rehabilitation patients comfortable with the teach back method and the structure of education? 2. To what extent did the intervention impact the phase II cardiac rehabilitation patients knowledge of their individual cardiac health and confidence in themselves to maintain their improved cardiac health into phase III? 3. How did this intervention impact my co-workers time management with their responsibilities? This study's methods, results, implications and conclusions are expected to inform best practices in phase II cardiac rehabilitation and contribute to the development of more comprehensive, supportive care models that integrate physical, mental, and social health.
