요약Diseases carried by ticks pose a large threat to the population's health in the United States of America. To achieve a more complete understanding of tickborne illnesses in the United States, this dissertation describes and characterizes the hospital burden of four major categories of tickborne diseases: Lyme disease, ehrlichiosis/anaplasmosis, babesiosis, and tickborne spotted fevers. Discharge records approximating 20% of all discharges from US community hospitals in the USA from 2012 to 2019 were obtained from the National Inpatient Sample (NIS). The first aim of this dissertation is to provide survey-weighted estimates of the counts and survey-weighted, age-adjusted rates of tickborne illness hospitalizations in the USA across the study period for each of the disease categories, as well as to provide counts and rates among strata of demographic and institutional variables. This aim found that after adjusting for changes in the age structure of the US population from 2012-2019, the rate of tickborne illness hospitalizations decreased over the study period for three of the four tickborne disease categories (Lyme disease, ehrlichiosis/anaplasmosis, and spotted fevers). Lyme disease, ehrlichiosis/anaplasmosis, and babesiosis hospitalizations all saw sharp increases in 2013 and 2017. From 2012 to 2019 there were an estimated 86,205 (95% CI: 83,732 - 88,678) hospital discharges with Lyme disease, 23,610 (95% CI: 22,509 - 24,711) hospital discharges with ehrlichiosis or anaplasmosis, 11,450 (95% CI: 10,829 - 12,071) hospital discharges with spotted fever rickettsioses, and 10,215 (95% CI: 9,593 - 10,837) hospital discharges with babesiosis. The rate of hospitalization from tickborne diseases were highest among older, male adults. The second aim of this dissertation examines demographic and institutional characteristics that predict indicators of more severe outcomes from tickborne diseases. The results from these analysis suggest that patient race, patient age, and patient sex are associated with potentially worse tickborne disease outcomes. The third aim of this dissertation uses demographic and institutional variables to predict the direct costs and lengths of stay for those hospitalized with a tickborne illness. Hospitalizations with ehrlichiosis or anaplasmosis had the lowest mean cost per hospitalization, while babesiosis had the highest. The total cost of hospitalizations from tickborne diseases across the study period exceeded 1 billion dollars. Overall, this dissertation provides context for the interpretation of national surveillance estimates for Lyme disease, ehrlichiosis and anaplasmosis, spotted fevers, and babesiosis.
