요약MS is a neurological disease that follows a heterogenous disease course across individuals, often impacting lower limb function, cognition, gait/posture, and fine motor function. Accurate measurement of function is essential for development of new therapies yet remains a challenge given reliance on infrequent in-clinic assessments which may not adequately capture function or subtle disease progression. Smartphone sensor-based assessment tools have potential to enhance the assessment of cognitive and motor function in neurological disorders by enabling objective, frequent, remote data collection. Sensor-based assessments can be used to generate vast amounts of data which raises a challenge for researchers when selecting sensitive and reliable digital measures. The current work utilizes smartphone-based assessment data from a cross-sectional study of people with MS and healthy participants to select sensor-based measures of motor and cognitive function for validation. Selected measures are characterized in terms of basic measurement properties. A systematic literature review focused on gait impairment in MS is also presented to develop a set of standardized gait domain definitions and recommended digital outcome measures to capture impairment in these domains. This work contributes to the field of digital endpoint development by characterizing the psychometric properties of smartphone-based measures of cognitive processing speed and motor function in MS, and by proposing systematic frameworks to aid in selection and validation of digital outcome measures in people with MS.
