- 요약This dissertation explores heart failure (HF) decision making to begin to understand how patients experience the course of their treatment decisions and outcomes. HF is a chronic disease that progresses unpredictably over time. Treatment decisions are made and processed over time, with multiple considerations and factors influencing the patient's choice and response to their decision. Treatment decisions are particularly challenging among HF patients, whose options represent high risk procedures with which patients attempt to align with their life goals and hopes for the future. Timely discussions between clinicians, patients and their caregivers are recognized as key to supporting patients. Gaining a deeper, longitudinally based understanding of HF decision making has the potential to allow clinicians to respond more purposefully to the needs of their patients.Included within the body of this work are three manuscripts. The first manuscript is both a systematic literature review and the results of a thematic synthesis conducted utilizing qualitative studies investigating heart failure decision making. Systematic review of the literature uncovered a growing body of studies exploring heart failure decision making, mainly of cross-sectional design, with few studies exploring beyond the initial point of treatment decisions. Thematic synthesis of existing qualitative studies revealed three themes common to decision-making stakeholders and across heart failure treatments. These findings led to a qualitative exploration of HF decision-making trajectory through the use of narrative data collected through interviews, and lifelines, which provided visual representations of each participant's decision-making trajectory. To triangulate and analyze these data sources, we utilized Sort and Sift, Think and Shift (Sort, Sift) methods. The second manuscript presents the methods, recommendations and lessons learned from conducting this explorative investigation. Results of this inquiry are presented in manuscript three. Ten patients who had decided to undergo LVAD implantation and subsequently received cardiac transplantation were asked to share their treatment decision-making experiences. From this work, five themes and nine subthemes emerged, which described and illustrated decision making over time and the key events and factors that influenced the patients' experiences. The concluding chapter synthesizes the findings of the overall body of this work. The implications of this work for nurses and other clinicians, as well as suggestions for future research in heart failure decision making, are also provided.
