- 요약Purpose: Nursing handoff is one of the most important points of communication in healthcare. Incomplete or incorrect handoff information can affect patient safety, continuity of care, length of stay, and costs for hospitals and patients. This project aimed to standardize the handoff process by implementing the I-PASS handoff tool within an adult psychiatric unit of an academic medical center. Methods: Baseline data was collected to establish the current rate of handoff errors as well as nursing perception of handoff quality. The I-PASS tool was implemented on the unit and data was collected post-implementation to evaluate the effectiveness of the tool in decreasing handoff errors. Results: 18 nurses participated in the implementation of the I-PASS tool. There was a decrease in handoff errors from 20% at baseline to 15% post-implementation. Majority of participants reported post-implementation that the tool was easily incorporated into their practice and that the tool was helpful in reducing handoff errors. Conclusions: This project demonstrated that handoff errors are a point of concern for nurses working on an inpatient psychiatric unit. Standardizing the process assists in decreasing the number of errors that occur during shift handoff.
