- 요약Background: Music interventions have demonstrated benefits for motor recovery, emotional well-being and neural reorganization to support stroke rehabilitation. However, heterogeneity across music interventions limits understanding of the mechanisms that drive improved outcomes. Improvisation and live accompaniment have demonstrated positive effects on motor performance and reward-related responses; however these effects and their neural mechanisms have not been evaluated in survivors of stroke. Objective: In this study, we evaluated motor, autonomic, affective, and neural responses during a drum playing task in which participants either (i) improvise a rhythm or maintain the beat and (ii) play with live or recorded accompaniment. Methods: Sixteen survivors of stroke and sixteen neurologically normal control (NC) participants completed the study. Motor responses were characterized by acceleration of hand movements (accelerometry) and wrist flexor activation (electromyography). Autonomic response was evaluated with tonic response of electrodermal activity (EDA). Affective responses were evaluated with 5-point Likert ratings of pleasure, physical effort, and cognitive challenge. Neural response was evaluated in a subset of 11 stroke and 11 NC participants with fMRI, using region of interest analysis to evaluate motor and reward-related brain regions including the supplementary motor area (SMA), precentral gyrus (PCG), and ventral striatum (VS). Principal component analysis of all behavioral and physiological measures confirmed that EDA and pleasure ratings loaded onto the same component, orthogonal from components related to motor response and ratings of challenge. The component with loadings for EDA and pleasure was used to evaluate the association between motor and reward-related responses.Results: Improvisation increased acceleration of hand movements, wrist flexor activation, tonic EDA, and activation in the SMA, PCG and VS. Improvisation and live accompaniment increased pleasure ratings. Reward-related responses were positively associated with both acceleration of hand movements and wrist flexor activation. Survivors of stroke had higher ratings of physical effort and cognitive challenge compared to NC participants.Conclusions: These findings support the benefit of improvisation for improving motor response and reward during music playing among survivors of stroke, while live accompaniment increased pleasure ratings. The positive association between reward and motor responses suggests an important role of reward for improving motor performance among survivors of stroke, which suggests that optimization of reward during music playing may have beneficial effects for enhancing motor recovery outcomes.
